This means, although the correct code has been entered, you have used up all of your five unlock code attempts and therefore the phone will not allow the unlock.

No unlock code will ever work for you now... you need a cable unlock solution like Griffin or Neelix. Or you could take to a shop that has such equipment and ask them to unlock it for you...
