Some time ago modifying in HEX editor the code *#0000 I noticed such a code *pw+. After many tests and attempts I stated that it is code to locking phone.
It works this way:
At the beginning it is necessary to clear all locks in Nokia tool by Rolis.
So...
In Knok "Modify Locks" --> "MCC+MNC" - choose setup: "Open", value: current country code+network code (MCC+MNC), type: "User", press "OK" and restart the phone.
Next enter the PIN code and enter the code *pw+MCC_MNC+1# (the one which was entering in Knok) - the phone will display message "SIM restriction on", then it will restart and lock will be closed.
It is also possible to close lock automatically:
In Nokia tool by Rolis read the phone and at the first lock change the status on "AUTO/FACTORY", press "OPEN" and restart the phone, enter the PIN code - phone display the message "SIM restriction on" and automatically will restart closing lock on the network which the SIM card is in a phone.
With these methods it is possible also to close lock MSIN (4th lock) and what is the most interesting in this case Nokia DCT-3 can serve for reading IMSI of the SIM card :)
So...

read the phone again, change the status of the 4th lock on AUTO/FACTORY and press "OPEN".
Next restart the phone, enter the PIN code - phone display the message "SIM restriction on", then it will restart automatically.
Next read the phone with Nokia tool.
Ahead of this number we put the country code and network code and we have the IMSI of the SIM card :)
From that moment the phone will work only with this SIM card (not to confuse with the security code)
4th lock it is possible also to close with code *pw+MCC_MNC+4# this way as it I described above but...

Next in "Modify Locks" --> "MCC+MNC" - choose setup: "Open", value: current MCC+MNC, type: "User" press "OK" and restart the phone.
Now is possible to close 4th lock on SIM card with code *pw+MCC_MNC+4#
