Help unlocking nokia 7650

I have tried to unlock a nokia 7650 from UK network t-mobile and have tried the code

#pw+833621251106046+7#

it come back code error,

can anyone help or advise why this is happening

from the minimal amount of info given it could be

1) you did not pick Asic 5 as the phone type
2)you did not choose V2 as box serial
3) you did not get the IMEI from the phone itself but the box and the box is wrong (type *#06# to check it on the phone)
4) you might have to try code 1 and then code 5
5) is it definately locked to T-Mobile or you taking someone elses word for it ??
