Has anyone been successful at unlocking the Nokia 7610b (RH-52) locked to Cingular (USA) using the software version "V 4.0440.02 01-10-04 RH-52" ?

Here is my phone info:

Model: Nokia 7610b (RH-52)
Locked To: Cingular (USA)
IMEI: xxxxx
Software: V 4.0440.02
01-10-04
RH-52

I have tried several settings already, up to 4 tries, but no luck yet!!! Here are some of the settings that I have used thus far:

MCC+MNC: 31017000
Type: Asic 2
Box Serial: T-Mobile 32802
V2 33820
Code: 1st, 7th, 2nd.....

Any help is appreciated, especially if anyone had success at unlocking with this exact same model and software version.
