need help unlocking 5165 NSW-1NX from ATT

Hi all:
1. what type is this phone - the FAQ says it is TDMA, but some other list it in DCT3 sectio - may I assume it it DCT3?
2. it does not respond to #*06# and does not have IMEI number on the back, only the ESN. How do I find the IMEI?
3. If I enter the ESN into the unlock calculator on this site, it comes back with a code, which when entered does not produce any results. The code that comes back contains one blank space in front of +1# - how do I enter the space?

All the info I found on unlocking this phone so far is extremely confusing. Please help!

Sorry, it was a typo in my original posting: I am using *#06# and nothing happens. Does the phone have to be in service? It is not cause I deactivated once the contract was over. And it is not fully charged at the moment, maybe that is why?
Another surprise: the codes like #pw+1234567890+1# etc do not do anything, I understand that they should show the lock status but nothing happen. What am I doing wrong?

the unlock codes must be inputed in the phone without sim card

I have a Nokia 5165 which I beleive is a tdma phone. It appears to be locked to ATT. I am told that unless you already have an old ATT account that you cannot activate a new phone with them. Since they merged with Cingular all new activations must be made with Cingular. Apparently the two systems have not completely merged. Cingular now uses GSM. I need to get this phone unlocked so I can use it on another tdma network since apparently it cannot be used on Cingular. I cannot seem to find unlocking codes for the tdma or cdma phones for that matter either. All I can find is unlocking for GSM or SIM based phones.

Can anyone direct me to a website where I can get the unlock code for this phone. Any help would be appreciated.

The 06 code won't work for me on this phone. I need to get this phone unlocked or the phone will be useless.
