Just download Codes Calculator v3.10
click on "options" button:
Select "IMEI checksum verification"
Select "Expert" mode (Under the Nokia session)
then click back

on the calculator screen
Select Nokia
DCT4 (expert only)
IMEI: "type your IMEI number here", you can get it by press *#06# at your cell phone.
MCC+MNC (don't change this field)
USA (location)
T-mobile (Provider)
ASIC 2
T-mobile

At your cell phone, you need to insert a SIM card into the phone before powering up.
It will say restricted ....
You should then press #pw+298795113607013+1#
to get to "p", "w", "+" press "*" several times

It will then unlock
