As a quick guide. Go to the unlock codes section and download the nfree calc standalone version, I think it is v3.10. Install it, go to options, and set the calc to expert mode. In the dropdown model list, select "DCT4 Expert Only". Select your country and network, enter your imei and for the serial, there are some options.

You can use T-Mobile (32835), which has worked with T-Mobile handsets, Original (for almost all networks but since may 05 it may has been blacklisted), ASIC6 (32821) or ONE (32836). Use the search engine linked above to find out which is your best option.

For the Asic Type, your phone is Asic Type 5.

Of the seven codes list, use code #1.
