5140 Cannot undo restriction

Why will this my Nokia 5140 not unlock from the Orange (UK) network? I have carefully followed the instructions for using the calculator and entered the codes as described - the only response I get is the "Cannot undo restriction". What could be wrong - someone please help!!

Wot cables do I need, wot software do I need and where do I get them from.

Cables can be pretty expensive if you only plan to unlock one phone. Just go to a phone shop and get it unlocked for £10 - £15. Easiest option.
