Have someone got a operator list form?

Hi, I'm going to make something really cool for all of you.

But there is one thing i need - a form with all the operator names in a dropdown list - and the "[b]option value[/b]" to the operator name must be the operator code.

Like this:

<option value="26801">Country Operator</option>

If someone have a list in this format, could you please tell me?

Note: The important is that the [b]option value[/b] is only the code - not the name of the operator also, like the form you find here has: [url]http://ucables.com/remote-unlock-online[/url]

Hope someone could help me - it will be really cool for all of you (hint: superfast free code generation...)

I could of course make this form field myself, but it would take to much time to put in the operator code.
