Unlock code needed for 3650

I have a cingular Nokia 3650 and need an unlock code for it.
IMEI # 351102/50/047691/5
The phone was bought in LA. I'm a newbie so do you need to remove the simcard to enter the code. If someone can help me it would be much appreciated.

I am definitely Cingular. What happens if I have had more than 5 goes, does it completely lock up?
