when you unlock do you keep your sim card out?

when you are pressing the unlock code do you do it without the sim card in the phone or with it in the phone?
Can someone help. I can't unlock my phone.
Also, is the netcode for Cingular Washington USA 310 17?
I got codes for my using netcode 310 17 but it didn't work
Can a kind person give me unlock codes by using Netcode# 310 15 instead?
