I've done some reading, and the most consistent verified results come from using the SmartDCT4 calc with V2 checked and Code1 used.

2.) Input your IMEI# along with code (use drop down box)

3.) Code Type V2

4.) Calculate Unlock Codes!

5.) Input the Code 1 (the one with the +1 at the end) onto your phone.
